How Much Does Caffeine Raise Blood Pressure?
A dose of 200 to 300 mg of caffeine, about two to three cups of coffee, raises your top number (systolic) by about 8.1 mm Hg on average¹. Your bottom number (diastolic) rises by about 5.7 mm Hg on average. Both figures come from trials in people who already have high blood pressure.
A separate review looked at caffeine supplements rather than coffee itself. It found a smaller effect: 1.94 mm Hg on the top number and 1.66 mm Hg on the bottom number². That is a lighter rise, but it still held up clearly across the pooled studies.
The two reviews are not measuring quite the same thing. One studied coffee in people who already had high blood pressure, the other studied caffeine supplements in a broader group, which is part of why the numbers differ¹².
Either way, the rise is real, and its size matters because of the goal your doctor is working toward. Current guidance sets the blood pressure treatment goal at under 130/80 mm Hg for adults³. A rise like the one above can be enough to push a reading past that goal for a few hours.
This goal does not loosen as you get older. It stays the same at every age⁴. An average reading climbs over the years, but the target does not move just because you have.

How Long Does a Caffeine-Related Blood Pressure Rise Last?
The rise starts within 30 minutes of a caffeinated drink and peaks after about an hour⁵. From there, it fades slowly rather than all at once. The exact pace depends on whether you already have high blood pressure.
If you already have high blood pressure, that pattern stretches out further. Your top number stays raised for the entire three hours one study measured⁶. Your bottom number stays raised for those three hours as well.
In people without high blood pressure, the bottom number eases sooner, about 90 minutes after that first cup⁶. The top number is slower to settle across the board.
A separate review of trials, looking specifically at people with high blood pressure, backs this up. A single dose keeps blood pressure raised for three hours or more⁷. Two different studies now point to the same three-hour window.
So a cup with your breakfast can still be shaping your reading well into the late morning. The rise is real, but it does settle back down on its own. It just takes longer than you might expect, especially if your blood pressure already runs high.
Why Does Caffeine Raise Your Blood Pressure?
Caffeine seems to affect your body in a few different ways that push your numbers up⁸.
- It may block a hormone that normally keeps your arteries widened, so they narrow instead.
- It may prompt your adrenal glands (sitting above your kidneys) to release more adrenaline, a hormone that raises blood pressure on its own.
- Everyday stress may also play a part.
Scientists have not settled on which of these matters most. The exact mechanism behind the rise is still an open question⁸.
In animal studies, caffeine has also been linked to nerve activation (your body's fight-or-flight response) that raises blood pressure in a similar way⁹. That same animal research found a brief jump in activity and heart rate alongside the rise in blood pressure, not blood pressure alone. Several different systems in your body seem to react to caffeine at once, which is why researchers describe more than one path to the same result.
Does Everyone React to Caffeine the Same Way?
Two things explain why caffeine hits people differently: tolerance and genes.
If you drink coffee or tea regularly, your body adjusts. The short-term rise in blood pressure shows up mainly in people who do not drink caffeine often, not in regular drinkers¹⁰. Even then, the blood pressure response to caffeine varies from person to person.
How fast your body breaks caffeine down is partly written in your genes. A liver enzyme called CYP1A2 handles the breakdown, working faster in some people than others. If yours works slowly, more caffeine stays in your system and your reading climbs further.
One study found that people with one gene type started with a higher reading. After caffeine, their reading climbed even further, while people with a different gene type barely changed at all¹¹.
A randomized trial put a number on that difference. People with one gene variant saw their top number rise by 11.8 mm Hg after caffeine. A different variant produced a smaller rise, just 4.1 mm Hg¹².
So if one cup leaves you jittery while someone else drinks four without a flicker, it is not in your head. It comes down to how often you drink caffeine and how your body is built to handle it.
Coffee and Blood Pressure: What Happens Over the Long Term?
Long-term studies do not find a higher risk of high blood pressure in people who drink coffee regularly. If anything, the research points the other way. Drinking coffee day after day is a different story from having one cup on one afternoon, and it is not a habit you need to worry about long term.
Two large analyses used cohort studies (research that follows the same group of people for years) to see who develops high blood pressure and who does not. One pooled data across many such studies and found the risk fell by about 2% for each extra cup of coffee a day¹³. That pattern held steady even at higher intake, with no sign it reversed.
A second review combined 13 cohort studies that followed 314,827 people in total. It found no meaningful link between how much coffee you drink and your risk of high blood pressure¹⁴.
The result ranged from a small decrease to a small increase in risk: 0.90 to 1.05. That range is what statisticians call no clear difference¹⁴.
Across years of data, coffee itself adds no extra risk to your blood pressure.
How Much Caffeine Is Safe if You Have High Blood Pressure?
For most healthy adults, the FDA cites 400 mg a day, about two to three 12-ounce cups of coffee, as an amount not generally associated with adverse effects¹⁵. That is a ceiling, not a goal to aim for.
If you already have high blood pressure and drink caffeine regularly, you likely do not need to limit or stop it¹⁶. If you are not currently a coffee or tea drinker, you do not need to start: people who are new to caffeine see the biggest rise, not people already used to it¹⁰.
Caffeine is only one piece of the picture. There are broader ways to bring your numbers down.
How Do Decaf Coffee and Energy Drinks Compare to Regular Coffee?
If you switch to decaf, expect a reading close to what a caffeinated cup gives you. Energy drinks push your numbers further, and the heaviest single doses push furthest of all.
In a trial of 104 people, the top number differed by 2.77 mm Hg between decaf and caffeinated coffee, right at the edge of significance¹⁷. The bottom number differed by 2.11 mm Hg, not a significant difference. Decaf behaves almost like the real thing.
Energy drinks are a different story, mostly because of how much caffeine they pack in. An 8-ounce cup of regular coffee typically has 80 to 100 mg of caffeine¹⁸. A 16-ounce energy drink typically has 70 to 240 mg¹⁹, more than double a cup of coffee at the high end.
A review of energy drink trials found that a single drink raised the top number by about 4.44 mm Hg. It raised the bottom number by about 2.73 mm Hg²⁰. Heart rate, measured in the same trials, did not change by a meaningful amount, about 0.80 beats per minute.
The biggest rise showed up with the biggest doses. Drinks delivering 200 mg of caffeine or more raised the top number by 6.44 mm Hg on average²¹. The same logic holds if you stack doses: several cups of coffee, or more than one energy drink back to back, add up the same way one heavy dose does, and your reading climbs with them.

Does Caffeine Affect Your Blood Pressure Medication?
Coffee can change how well a blood pressure medication works. How much depends a lot on the person.
One study tested coffee together with felodipine, a calcium-channel blocker used for blood pressure. This kind of study is called hemodynamic, meaning it tracks blood flow and pressure. The coffee in the study had 127 mg of caffeine²². Together, the coffee and the drug caused the biggest rise in blood pressure seen anywhere in the study.
That rise was not the same for everyone. It changed a lot from person to person²². The coffee did not just speed up how fast the body absorbs the drug. It changed how the drug worked in the body.
The effect is different for each person, so this is worth asking about directly. Ask your doctor or pharmacist whether coffee timing matters for your medication. Tell them how much coffee you drink and about when, so their answer fits you.
How to Check Whether Caffeine Is Raising Your Own Blood Pressure
Run a before-and-after check²³. This shows whether caffeine, specifically, is behind a change in your reading.
- Take a reading before your coffee or other caffeinated drink.
- Take a second reading 30 to 120 minutes later.
Compare the two numbers. If you don't normally drink coffee and your second reading is about 5 to 10 points higher, you may be sensitive to caffeine's effect on your blood pressure²³²⁴.
That is a fact about your own body. It tells you more than any group average could.
If that result leads you to cut back, taper slowly rather than stopping in one day. Spread it over a few days to a week, so you avoid the headache that sudden withdrawal can bring²⁴.
One result calls for more than jotting it down. If a reading comes back higher than 180 and/or higher than 120, wait one minute and take it again. If the second reading is just as high, this needs urgent medical attention²⁵.

Why Do You Need So Much Caffeine?
If you feel like you cannot function without large amounts of caffeine, that feeling is worth listening to, not overriding with another cup²⁶. It is worth asking yourself why you need so much.
Start with sleep. Are you getting enough of it, most nights? Stress can drive the same craving for caffeine²⁶.
